Prepare a quiz handout and separate teacher key.
Bring your questions, four choices and supplied answers. Prepare a printable student handout and a separate teacher answer key, with text you can copy into your own document.
Google Forms already supports quiz answer keys, and you can prepare a handout and key manually in Google Docs. Use those if they meet your needs. This formatter requires manually prepared or selected columns; it does not extract or export a Google Form, grade responses, send files, import anything or connect to an account.
Your title, questions, answers and generated files stay in this browser until you copy or download them. Usage events contain fixed labels only, never your quiz content. No uploaded or saved quiz records.
1. Bring your quiz questions and supplied key
Prepare the exact CSV columns question,a,b,c,d,answer, points. Each question needs four nonblank, distinct choices. Use A, B, C or D for the answer and an integer from 1 to 100 for points. Supply 1–25 question rows. Quote values containing commas, quotes or line breaks using normal CSV rules. Select these columns yourself; this is not a Google Forms export format.
Limits: title 120 Unicode characters, question 1,000, each choice 500, and whole CSV 60,000 characters. Remove leading and trailing whitespace from title, questions and choices. Choices must remain distinct when ignoring case and equivalent Unicode composition. Invalid input is rejected in full.
Own input mode. All fields start empty.
3. Check the source, layout and sharing yourself
Cross-check every question, choice, answer and point value against your original source. The supplied answers are formatted as given; correctness and suitability have not been verified. Review the student file separately for accidental answer disclosures in your own question or choice wording.
Open each saved HTML file locally and check actual print preview, page breaks and layout before printing. If you copy text into Google Docs, review the pasted layout and create separate student and teacher documents with appropriate sharing settings. Copying, printing and sharing remain your steps.
